A founder's story of fragmented care — and a mission to fix it.
In 2024, my life changed overnight. I developed Stevens-Johnson Syndrome, a rare condition that nearly took my life and left me having to relearn how to breathe and walk independently while facing the terrifying reality of vision loss.
But the hardest part wasn't just the illness—it was the silence between my doctors. I spent weeks in hospital beds, saw specialist after specialist, and went to the ER three separate times before anyone finally connected the dots. Each time, I had to start my story from scratch because the systems meant to protect me didn't talk to each other. I felt invisible in my own crisis.
I survived and got back my vision, but many people don't. And many more spend their lives exhausted by a healthcare system that only sees them in fragments. I started TraceBack because no one should have to fight that hard just to be heard, and no doctor should have to make life-saving decisions in the dark.
— Chloe Mombeshora, Co-Founder
People are falling through the cracks of a system that treats them as episodes, not individuals. TraceBack bridges the gap between appointments — so clinicians see the full picture, and patients don't have to fight to be seen.
